Registration Terms of Agreement & Refund Policy for CCCE Trainings

These terms and refund policy are repeated within the registration form, and MUST BE SIGNED in order to submit your registration. There are no exceptions to any of the following.

REFUND POLICY:

If Tiffany Miller cancels the training, attendees will be offered a full refund or a one-time transfer to another CCCE training by Tiffany Miller.

There are no refunds for this training if the attendee cancels. If the attendee cancels more than 3 weeks prior to the training, she will be offered a one-time transfer to another training date (see below).

Should an attendee wish to transfer to another training, she must receive written approval from Tiffany Miller. Only one transfer is allowed, it must be within 12 months of the original training date, and the attendee will be charged a $50 transfer fee.

If an attendee cancels less than 3 weeks prior to the training start date, she will not be eligible for a transfer and will not receive a refund.

Should an attendee fail to transfer to another training within 12 months and/or fails to pay the $50 transfer fee, all fees are forfeited.

If an attendee does not show up for the training, they are not eligible for a training transfer, and will not receive a refund of any portion of the training fees.

Attendees should not make any non-refundable travel reservations until Tiffany Miller has confirmed the training, usually at least 2 weeks prior to the training start date.

In the event of illness, emergency, hazardous weather, or any travel issues making it impossible for Tiffany Miller to hold the training, a makeup option will be made available, or attendees may transfer to another training within 12 months, and the transfer fee will be waived. Tiff is not responsible for any travel expenses attendees may incur in this event.

TERMS OF AGREEMENT:

I understand that I must be at least 18 years old to take a CAPPA training.

As a participant of this training, I agree to allow my CAPPA Faculty to publish my name, phone number, and email address for the student class list and referral list.

As a participant of this training, I agree to allow my CAPPA Faculty to take photographs of the training which may include me, as well as allow the CAPPA Faculty and CAPPA to use these photographs for promotional purposes, online and in print.

As a participant of this training, I agree to allow my CAPPA Faculty to take video/audio recording of the training which may include me, as wall as allow the CAPPA Faculty and CAPPA to use the recordings as they wish.

As a participant of this training, I agree to allow my CAPPA Faculty to use quotes from my training evaluations (which I may fill out anonymously) for promotional purposes, online and in print.

 

I understand that I may not record the training (audio or video).

I may not photograph any PowerPoint slides, training materials, certificates of completion, or any other intellectual property belonging to CAPPA or the faculty.

I may only take and/or post photographs of the CAPPA faculty and training participants only with their express permission. Photographs are for my personal use only, and may not be shared.

I may not reproduce, publish, photograph, share, email, or otherwise distribute any materials in the training handouts or manual, other than for the purpose of fulfilling my certification requirements (i.e. copying the evaluation forms to be filled out).

I understand that babies/children are not permitted at the training. Nursing babies are welcome during lunch. Off-site childcare arrangements are my responsibility.

A certificate of training completion will be given only if I attend and fully participate in the entire training and the training fees are paid in full.

I understand that there will be mandatory homework assignments (such as a study guide) that I will need to complete prior to the training.

I understand that the two-day training alone does not complete my certification with CAPPA.

I understand that this training is only one step in becoming a CAPPA Certified Childbirth Educator. There are several other steps that I must complete (requiring an additional investment) after the training to be eligible for certification by CAPPA.

I understand that using the titles CCCE or CAPPA Certified Childbirth Educator (or any variation thereof) prior to officially earning my certification from CAPPA will result in being disqualified from being certified.

I understand that I must enroll in CAPPA Academy to complete my certification requirements, receive instruction on completing certification, download forms, and take my certification exams. Enrollment in CAPPA Academy requires an additional investment.

I understand that should I choose to complete the certification process, I must be a CAPPA member. Membership can be purchased at www.cappa.net, or in person at the CCCE Training.

I understand that I will receive an email confirmation from Tiff Miller within two business days of submitting this form. If I do not receive that confirmation, I will contact Tiff at birthinjoy@gmail.com to ensure that registration was received.

I understand that I am not officially registered for this training until I have completed this registration AND the payment for the training fee has been received by Tiff Miller.
